Ownership Change

Listed below are some common situations that result in a change of ownership. Please note the appropriate situation and forward the requested documents to JP Oil Holdings, LLC. Please also include the new payee’s full name, address, phone number and social security number. Ownership changes will be implemented in the system as soon as possible upon receipt by JP Oil, however, we must receive all of the appropriate documents by the 10th of the month to ensure the ownership change is made before the following check run.

SITUATION PROVIDE THE FOLLOWING DOCUMENTS
Change in Ownership Due To Death of an Owner (died intestate or without a Will) • Copy of death certificate;
• Death and Heirship Affidavit. (  Download Affidavit Form);
• A complete and signed W-9 for each heir (  Download W-9 Form)
Change in Ownership Due To Death of an Owner (died testate or with a Will) • Copy of death certificate;
• Copy of Last Will and Testament and Final Decree of Distribution/ Judgment of Possession, or Order Admitting Will to Probate;
• Letters Testamentary;
• A complete and signed W-9 for each heir
Sale of An Interest • Copy of recorded instrument (deed, assignment, or other conveyance instrument), recorded in the county or parish and state where the minerals reside.
Transfer of Ownership To A Trust • Copy of Trust Agreement or Memorandum of Trust along with recorded conveyance in appropriate county or parish and state where the minerals reside;
• A complete and signed W-9
Name Change A photocopy of marriage certificate, divorce decree, social security card, driver’s license, or any document showing that the name has been legally changed.
Change In Ownership Due to Divorce • Complete Divorce Decree including any conveyances made and filed of record in the appropriate county or parish and state where the minerals reside;
• A complete and signed W-9
Company or Corporation’s Name Change • Certificate of Name Change;
• A complete and signed W-9
Company or Corporate Merger • Certificate of Merger;
• A complete and signed W-9
Appointment of Power of Attorney or Guardian Copy of Power of Attorney or Letters of Guardianship
Bankruptcy – Chapter 7 Order naming Trustee of Bankruptcy Estate and appropriate recorded conveyance.
